Immigration Clinic

The Immigration Clinic will provide the necessary documentation for you to provide to the U.S. Citizenship and Immigration Service for a change in your immigration status. The documentation requires at a minimum:

  • Review and update of “age appropriate” immunizations
  • Skin testing for tuberculosis, if older than two years of age
  • Blood test for syphilis, if older than 15 years of age
  • Review of past and current health issues
  • Physical examination


IMPORTANT INFORMATION

  • Payment - Pre-payment for your exam will be collected at your appointment. The cost varies with age; for example, if you are 18 years old, your pre-payment fee is $225. Charges incurred for any additional testing including, but not limited to, blood tests, x-rays and immunizations are your responsibility and will also need to be paid at time of service.
  • Patient Accounts will call you prior to your appointment to review your insurance benefits and provide you with the amount you will be required to pay at the time of your appointment. Payments can be made with cash, check or major credit card (VISA, Mastercard or Discover). Please note: if you pay by check, you need to pay 10 days prior to your appointment. If you pay by check the day of your appointment for charges incurred that day, your document will be held for at least 10 days. You will not receive a completed document until all charges are paid in full.
  • Time frame - Please plan ahead when scheduling your appointment to ensure you receive your exam in time for the citizenship process. We will provide service to people in the order requested. There is no guarantee for the time this process will take for each individual.
  • Waivers - Some immunizations may be waived if an applicant is pregnant or allergic to components of the vaccine. The pregnancy waiver expires at the time of delivery.
