The Carle Subacute Center
The Carle Subacute Center is a 42-bed unit fully licensed by the State of Illinois and is Medicare certified.
Carle began offering subacute services in February, 1997, in conjunction with the Illinois Department of Public Health Subacute Care Hospital Demonstration Project. The Carle Subacute Center was developed for people who no longer require hospitalization, yet still need comprehensive inpatient medical care or rehabilitation before returning home.
Carle is one of thirteen facilities participating in the Illinois Department of Public Health Subacute Care Hospital Demonstration Project. This project was developed to assess the viability of this level of service as well as its cost efficacy. Carle has been participating in this demonstration project for more than two years, and now offers the first program of its kind in Central Illinois.
Subacute services are provided by a team of skilled professionals, including physical, occupational, and speech therapists, registered nurses, pain management specialists, nutritionists, social workers, counselors and discharge planners. This team works together to design a plan of treatment which addresses the care needs of each patient, promoting a quick recovery and return home.
Admission Criteria
- The patient may have significant, continuous, complex medical problems that necessitate close monitoring but no longer require an acute care setting.
- Patients must be clinically and physiologically stable.
- The patient must have realistic potential for functional improvement through rehabilitative services and/or must require multiple and complex medical/nursing interventions.
- The patient must have a realistic plan for discharge to an appropriate setting (i.e. home, assisted living, long-term care).
- The patient must be at least 18 years old.
- If agitated, patients must have controlled agitation.
Program Highlights
On-site physician visits available daily, if needed, including weekends.
RN clinical nurse manager and RN staff nurses 24 hours a day, seven days a week.
Physical, occupational and speech therapy seven days a week.
Admissions taken seven days a week.
Admission Process
Call our admissions coordinator at (217) 383-3090 and request placement at The Carle Subacute Center.
Call in a report to the Carle Subacute Center physician prior to admission.
Physicians may follow their own patients or may call (217) 383-3001 and request the Carle Subacute Center on-call physician, 7 a.m. - 5 p.m., seven days a week.
