Carle Foundation Hospital's Patient Accounting and Health InformationManagement Departments Move to New Location

Two departments ofCarle Foundation Hospitalhave moved to a new location. The Patient Accounting and Health InformationManagement departments are now housed at 2902 Farber Drive in Champaign. This new location is in the InterstateResearch Park,near the intersection of I-57 and Olympian drive.

Patients requestingor submitting medical records can pay a walk-in visit to this location, Mondayto Friday, between the hours of 8 a.m. and 5 p.m. Depending on the nature andquantity of records requested, patients may have to wait several days for theirrequest to be completed. The necessary release forms can be downloaded onlineat www.carle.org/medicalrecordsor by calling (217) 383-3381. Completed release forms can also be submitted viafax at (217) 326-1801.

Patients who needhelp with their hospital bills, or who have questions about Carle FoundationHospital's Community Care Discount Program are encouraged to call the PatientAccounts department at (217) 326-2900 (toll free 866-473-6196) or stop by andtalk to a staff member between the hours of 7:30 a.m. and 5 p.m. Monday,Wednesday, Thursday or Friday, or on Tuesday between 7 a.m. and 7 p.m.
