Easing Your Transition
The Transitional Care Clinic (TCC) is for patients with complicated medical conditions that would benefit from additional support after leaving the hospital. The time after a patient leaves the hospital can be overwhelming and difficult. Patients may have a number of new medications to manage. Or, one may have new equipment to use, such as oxygen. Whatever the unique situation, the Transitional Care Clinic team is here to help. Our team works with patients to stay on the path to recovery after leaving the hospital until visiting with a Primary Care Provider.
Patients are evaluated and referred to the Transitional Care Clinic when they are in the hospital. If you’ve been referred to the TCC and have questions, please contact the TCC at (217) 365-8709.

Getting There
Park in the Orchard Street Garage. Go in the Main Entrance, walk through the lobby and turn left. Take the Blue Cloud Elevator to the second floor.
